How do I place my membership on hold or cancel it?

Membership cancellation form can be found here. Membership hold requests must be filled out 30 days prior to your next payment date. No refunds or completed.

Membership hold form can be found here. Membership holds can be completed for up to a minimum of 1 month and up 4 months per calendar year. You cannot exceed more than 3 separate holds. Membership hold requests must be completed 15 days before the next payment is due. A $20 hold fee will be charged to your card and will be taken off your first full months payment when you return.

Email chelseaschroeder@fit4mom.com to change to a different membership. All requests need to be received in writing 15 business days before the next payment is due.
